The clitoris
The visible part is the tip of a much larger structure. Most of the organ sits under the skin, wraps around the vaginal canal, and swells during arousal. Almost every practical question about technique follows from that shape.
For most of the twentieth century, anatomy textbooks showed the clitoris as a small external button. The full internal structure was described in detail in the late 1990s by the urologist Helen O'Connell, whose dissection work established that the organ is several times larger than the diagrams suggested and intimately related to the urethra and vaginal wall. That correction is recent enough that plenty of otherwise reliable material still repeats the older picture.
The actual shape
The clitoris has four parts. The glans is the visible portion at the front of the vulva, partly covered by a hood formed from the joining of the inner labia. Behind it, the body runs back and upward for a couple of centimetres. The body then splits into two crura, legs of erectile tissue that run several centimetres down and back along the pubic arch on either side. Flanking the vaginal opening are the two vestibular bulbs, further masses of erectile tissue.
Taken together, the structure is commonly measured at around nine to eleven centimetres in total, most of it internal. It is not arranged in a line but as a wishbone straddling the vaginal canal and the urethra.
What engorgement does
All of that tissue is erectile, which means it fills with blood during arousal in the same way and by the same mechanism as a penis. The consequences are practical rather than academic.
The whole area becomes more sensitive, not merely the glans, because the crura and bulbs sitting under the labia and around the entrance are now firm and full. Pressure applied through the labia, along the sides of the vulva, or at the perineum reaches erectile tissue that was not reachable before. This is why the area that responds well at minute twenty is much wider than the area that responds at minute two.
The hood also retracts somewhat as the glans swells, exposing more of it. That is why tolerance for direct contact increases as a session progresses, and why the same touch can be unpleasant early and excellent later.
Nerve density and why direct contact can be too much
The glans carries an exceptionally high concentration of nerve endings for its size, denser than any other structure in the human body. Density is not the same as capacity: a very high input to a very small, densely innervated area produces a signal the nervous system reads as overload rather than pleasure.
This explains a complaint that people frequently take as a personal fault. Direct pressure on the glans, especially before arousal, often feels sharp, irritating or numbing rather than pleasurable. That is a normal response to the anatomy. Contact through the hood, contact to one side, or broad pressure over the whole area distributes the input and is what a great many bodies prefer permanently, not merely as a warm up.
A second, related point: sustained high frequency vibration directly on the glans can produce temporary reduced sensitivity. It resolves within minutes to hours and causes no damage, but during a session it is usually mistaken for the body failing rather than for over-application. Moving off the point, reducing intensity, or switching to broad pressure resolves it.
The internal connection
Because the crura and bulbs surround the vaginal canal, pressure inside the vagina, particularly against the front wall, presses on clitoral tissue from the other side. This is the main reason the distinction between clitoral and vaginal orgasms is a poor one: the same organ is involved in both, approached from different angles. Practical consequences
- Approach the wider area before the glans, and treat the first several minutes as circulation rather than stimulation.
- Use lubricant on external touch. The glans and hood are mucosal at the edges, and dry friction there is the most common source of post-session soreness.
- Vary the point of contact rather than the intensity when things stall. Moving a centimetre is usually more effective than turning a motor up.
- If direct contact is uncomfortable at any level of arousal, that is a stable preference, not a stage to grow out of.
- Firm broad pressure with a flat hand, held still, is a real technique and not a beginner's move. It compresses the internal structures rather than abrading the surface.
Where toys fit
External vibrators, suction devices and wands all address this structure in different ways. Suction devices act on the glans and hood using pressure waves rather than contact, which is why they suit people who find direct vibration too sharp; the suction guide covers the mechanism. Wands deliver broad, low frequency stimulation that carries into the crura and bulbs, which is the reason they feel deep rather than surface level.
